Vessel Property Statement

i received a vessel property statement for 2006. where can i find an assessee to look over my boat to see whats its worth??? never had to do this before and im a bit confused!

Is it from the County of San Bernardino? if so you will get one every year....

i get one every year and its usually a bill. this time its a one page form asking everything about my boat. lenght, beam, draft, weight etc... at the bottom of the form it has to have a assessee sign it. how much does it cost to get an assessee? and where do i find one??
