You can thank the new emissions standars for that crap.
I saw that happen one time with a 95 power stroke 7.3L. One injector stuck wide open and 30,000lbs of injection will send alot of fuel into the exhaust. The spair tire melted from the radiant heat off the tail pipe. Spent nine years at a FORD truck shop.
