It's not necessarily a botch boob job. I have a few bras that are a bit, umm, let's say "undersized." When a bra doesn't fit right it gives your clevage a clevage. The bra material slices the mammory in half and suddenly four tits are on display through a tight tshirt.

Me thinks you are mistaken. An old hollywood trick of the trade is to put blush or some sort of makeup around the curve of your breast in your cleavage. It give the cleavage a greater 3-D effect and the breast appear larger. Jewel has employed this technique but it looks like she performed and her boobs settled farther into the dress than earlier in the evening, hence the odd looking photo. The make-up is clearly visible however. And for those of you hating on the tooth we tried braces but my pubes kept getting caught in them so we took them off.
